Thank you for your interest in this anonymous online survey study. This study involves minimal risk to you because no personally identifiable information will be collected. The goal of this study is to assess attitudes regarding various substances as treatments for Substance Use Disorders (SUDs) among individuals with a history of addiction. 

Volunteer Status
Your participation in the study is completely voluntary. You are free to withdraw from participating at any time. Even after you begin the survey, you may stop answering the questions at any time.

Procedure
Participation in this study involves filling out an online survey that will take approximately 15 minutes. At the beginning of the survey, you will be asked questions about your background and demographics (age, race, etc.). We will then ask about substance use and related treatment goals and history (if applicable). Finally, we will ask questions assessing your knowledge about potential risks, potential benefits, and overall attitudes about of the experimental drug being investigated, and whether you think it is an appropriate treatment for people seeking help with drug or alcohol use.

Inclusion Criteria
You are invited to participate in this survey if you fulfill all of the three criteria listed below.
1) You are at least 18 years old.
2) You read and write English fluently
3) You used to have a problem with drug or alcohol use but no longer do.

Confidentiality
Your identity and data will be kept completely confidential. Your data from the study will be coded with a randomly assigned identification number. No personally identifying information will be disclosed, even in the event of publication of this research. The data that you contribute will be kept in a secure location, and only the experimenters will have access to it. You will not be asked for any identifying information.

Why should I participate?
We believe that this study is scientifically important. We would like you to participate because we need to collect responses from many different people. You may find this survey interesting, and it is possible that the information presented during the survey might be helpful to you. Although there is no monetary compensation for participation, you will be making a unique and important contribution to science.

Risks/Discomforts
You may get tired or bored when you are completing the survey. Your participation in the study is completely voluntary. You are free to withdraw from participating at any time. Even after you begin the survey, you may stop answering the questions at any time.

What will become of the results from this study?
The researchers intend to publish the results from this study in the scientific literature and to present results at scientific meetings. We will also make the results publicly available by posting a notice of any scholarly publications on the website of the Johns Hopkins Center for Psychedelic and Consciousness Research (https://HopkinsPsychedelic.org), and make efforts to share with the community at large.

How do I start?

It is important that you complete the survey only once, and that you answer each question honestly and seriously. If you are not ready to complete the survey now, please return to this page at a time that is convenient for you. You can exit the survey at any time by closing your browser window. If you exit the survey early, your responses will not be used. During the survey, please do not hit the "back" button on your internet browser as it may erase your answers or prematurely terminate your session.
