This survey will ask about your child's experience returning to school following cancer diagnosis/treatment. We appreciate your help!
We will ask for your child's first name and age, but will not ask for any other identifying information about you or your child. In this way, your responses will be kept anonymous. By completing the survey you are agreeing to participate in a research study designed to help us better understand the return-to-school experiences and challenges of childhood cancer survivors and their caregivers.
The survey should take about 20-30 minutes to complete. You do not have to finish the whole survey at one time. You can continue the survey at any time as long as you are on the same computer and internet browser (e.g., Internet Explorer, Firefox, Google Chrome, etc.) on which you started the survey.
